Employer insurance and residential treatment leave run on separate tracks. One approval doesn't trigger the other. Your health plan decides whether it will pay for care. FMLA sets federal rules for eligible job-protected leave. The U.S. Department of Labor's employee guide to FMLA explains that eligible employees may receive up to 12 workweeks of unpaid leave during a defined 12-month period for their own serious health condition.
Insurance approval depends on your policy, network status, medical necessity rules, prior authorization, and ongoing clinical review. A complete FMLA certification generally requires enough medical facts to support leave, not your entire clinical chart. A diagnosis isn't automatically required. Read every authorization before signing it, and ask which records the employer or leave administrator is requesting.
Yes, employer policy may allow or require paid leave to run concurrently with FMLA. Ask HR how vacation, sick leave, short-term disability, and FMLA interact. Short-term disability policies replace part of eligible income and don't create job protection by themselves.
